Trash and Recycling Cart Order Form

  1. Instructions: Please fill out all parts of this form completely as it will help the Department of Public Works best assist you with your cart order. If you have questions, or need assistance with ordering, please call the Department of Public Works at 920-448-3535.
  2. Only the property owner may order trash and recycling carts for a property as the invoice for the cost of the cart(s) is sent to the property owner, not the tenant.
  3. Would you like a TRASH cart? *
    A household is only allowed ONE trash cart, per City ordinance.
  4. Would you like a RECYCLING cart? *
    A household may have multiple recycling carts.
  5. Check this box if you would like your old cart(s) collected for disposal.

    If you check this box, please leave cart accessible to crew.

  6. Property Owner*
  7. Payment

    Department of Public Works staff will send an invoice for your trash and/or recycling cart(s) to the property owner at the end of the month in which it was ordered.

  8. Delivery
    Carts will be delivered within 1-2 weeks, pending approval by Department of Public Works staff. If there are any issues with this request, staff will reach out to you directly.
